Your loved one might qualify for transition from a stroke, major surgery, joint replacement, or another hospital stay closer to home.
The Swing Bed program allows qualified patients to better transition from hospital (acute) care to home, assisted living, or long-term care.
A Swing Bed gives a patient the time and care needed for the following:
- Improve their strength and maximize their rehabilitation potential
- Get needed rest, physical therapy, and occupational therapy
- Complete their medication regimen
- Be assessed for performing activities of daily living (ADL)
